High SHBG: what it can mean for your testosterone

What a high result means

SHBG binds testosterone. When SHBG is high, more testosterone is held by it, so less is free. MedlinePlus says high SHBG suggests less total testosterone is available to your tissues, and that a total testosterone test can’t separate free from protein-bound testosterone. That is why a man can have a total testosterone result in the range and still have symptoms.

Whether your free testosterone is actually low is a separate question. It needs a free testosterone result, either measured or estimated from total testosterone and SHBG. See free vs total testosterone.

What has been linked to high SHBG

None of these are causes of your result until a clinician looks at your history. They are things that appear in the sources:

• Liver disease. MedlinePlus lists it. The Endocrine Society guideline lists cirrhosis.
• An overactive thyroid. Listed by MedlinePlus, and a 2025 review reports higher SHBG in women with hyperthyroidism.
• Certain eating disorders. Listed by MedlinePlus. The same review reports raised SHBG in anorexia nervosa.
• Aging. The Endocrine Society guideline lists aging among the conditions that raise SHBG. The 2025 review says SHBG rises in men after about age 40 as testosterone falls. MedlinePlus says the opposite, that it is normal for SHBG to fall with older age, so the sources disagree on this one.
• HIV disease. Listed by the Endocrine Society guideline.
• Decreased production of sex hormones in males. Listed by MedlinePlus.
• Estrogen-containing medicines. MedlinePlus lists them for women, for example birth control pills.

What a result can’t tell you

• It can’t tell you the cause. Several of the items above look the same on an SHBG report.
• It can’t tell you that your testosterone is low. For that, you need testosterone results and a clinician’s read of your symptoms. The Endocrine Society says a diagnosis rests on symptoms and consistently low results.
• Ranges differ by lab. A value just above the printed range is a different situation from one far above it, and only a clinician can say which matters for you.
