Do you need to fast for a testosterone blood test?

Fasting rules differ by test, and the answer for testosterone depends on what you plan to do with the result. This page gives the published guidance, what the seller’s pages say, and a helper that shows the preparation listed for each test.

What the guideline says

The Endocrine Society’s 2018 guideline for men with suspected low testosterone recommends measuring “fasting morning total T” as the first diagnostic test. It also recommends confirming a result by repeating the same measurement (Bhasin et al., 2018).

The guideline also says a diagnosis should rest on symptoms and signs as well as consistently low results. A single number does not make one. MedlinePlus says the same in plainer terms: a testosterone level test alone can’t diagnose any health condition (MedlinePlus).

Why eating can matter

One study gave 74 men a 75 gram sugar drink and drew blood for two hours. Mean testosterone fell about 25%, and it was still lower at the two-hour mark. Of 66 men who started with normal levels, 10 dropped into the study’s low range at one or more points. The authors concluded that men with a low non-fasting result should be re-tested while fasting (Caronia et al., 2013).

That was a sugar drink in a lab, not a normal meal, so it does not tell you how much breakfast would change your number. It does explain why a fasting draw is the version the guideline describes.

What the seller’s pages say

  • The testosterone therapy panel and the basic, standard and comprehensive male hormone panels: the seller’s page says fasting is required, with nothing but water for at least 8 hours.
  • The comprehensive panel’s page gives two figures: “at least 8 hours” in one place and “9 to 12 hours” in another. Follow your lab order.
  • The comprehensive metabolic panel also carries an 8 hour fasting rule on the seller’s page.
  • Single tests such as total testosterone carry a general fasting line that is shared across many tests. Do not treat it as advice for you. Follow your lab order.
  • Pages for IGF-1, SHBG, DHT, DHEA and sensitive estradiol say nothing about fasting. Follow your lab order.

Practical points

  • Fasting, in MedlinePlus’s wording, means no food or drink except water for up to several hours or overnight. The length can vary (MedlinePlus).
  • If you did not follow the instructions exactly, tell the lab. MedlinePlus says even a small change can affect results.
  • Tell your clinician about every medicine and supplement you take. The seller’s page for the comprehensive panel says to stop biotin supplements at least 48 hours before testing. Check with your clinician before stopping anything.
  • A morning appointment is part of the guideline’s method too. See the best time to test.

When to see a clinician

Talk to a clinician if you have symptoms that worry you, if a result is outside the range printed on your report, or if you take testosterone therapy. This page is general information and not medical advice.
