Repeat testosterone test: why a second morning draw is recommended
A second testosterone test is recommended before anyone concludes your level is low, and it does not mean the first was wrong. Hormone levels move, so one number is a snapshot.

What the guideline says
The guideline recommends diagnosing low testosterone only in men with symptoms and signs of deficiency and with “unequivocally and consistently” low results. It recommends measuring fasting morning total testosterone first, then confirming by repeating that measurement (Bhasin et al., 2018). A single low number is a reason to look again, not an answer.
Why numbers differ between draws
- Time of day. In a study of 66 men, testosterone in men aged 30 to 40 was 20 to 25% lower at 4 pm than at 8 am. Some men with a low reading in the afternoon had normal results at all three morning visits (Brambilla et al., 2009).
- Food. After a 75 gram sugar drink, mean testosterone fell about 25% in a study of 74 men, and the authors advised re-testing low non-fasting results while fasting (Caronia et al., 2013).
- Natural variation. A study of 132 men measured hormones on different days and months. For the hormones it studied, a clinician could expect to see a difference above 18 to 28% about half the time between two measurements, depending on the hormone. The authors concluded one sample is generally not enough to characterise a man’s hormone levels (Brambilla et al., 2007).
That last study covered several hormones. Its figures are not a rule for testosterone alone.
When a repeat comes up
MedlinePlus says a testosterone test may also be used to monitor your health if you have had an abnormal past result, take medicine that may affect testosterone, or take hormone therapy (MedlinePlus). The guideline above also describes a repeat to confirm a first result.
If you take testosterone therapy, your prescriber decides which tests you need and when. This guide does not set a schedule.
Doing a repeat well
- Book a morning slot. See the best time to test.
- Follow the fasting instructions on your order. See fasting.
- Keep your results together, with the date and time of each draw. Compare them against the range printed on each report, since labs differ.
- Choose the test that matches your question. If you want to compare the same thing, repeat the same test.
What a repeat can’t tell you
A repeat narrows the picture. It does not explain why a level is low or high, and it does not diagnose a condition. MedlinePlus says a testosterone level test alone can’t diagnose any health condition. See a clinician if you have symptoms or if repeat results stay outside the printed range.
